May I place a donation box in the medical library?

The library is happy to assist with donation drives sponsored by IU Indianapolis campus groups. However, after accumulating donation boxes that were no longer being maintained, we decided we needed to apply a policy to them.

A form needs to be completed and attached to the donation box to help library staff to determine when the donation drive is over and the box may be discarded.
Donation boxes not displaying the form will be removed, including any remaining donations. Forms are available at the Ruth Lilly Medical Library's circulation desk, IB 106.
